Updated Authorization for Sharing Health Information Form and FAQ
The PerformCare Authorization for Sharing Health Information Form and Frequently Asked Questions have been updated. This form is used to request protected health information (PHI) from PerformCare and allows PerformCare to share PHI with a designated person or organization.
Applications for CSOC Summer Camp Services are now available in English, Spanish, Portuguese, Chinese (Mandarin), Chinese (Cantonese), Korean, Gujarati, Arabic, and Haitian Creole. The application deadline is June 30, 2026.
